How to apply

The Kensington Hospice is an alternative to staying at home or in the hospital for end-of-life care. The hospice has 10 private rooms (with shared bathrooms) and is available to all adults living with a life-limiting illness. There is no restriction to a particular illness or condition.


Eligibility

In order to be admitted to the Kensington Hospice, you must be 19 years of age or older with a life-limiting illness and have a valid Ontario Health Card.


Admission

Becoming a resident at Kensington Hospice starts with a referral for palliative care by your family doctor, hospital or the Local Health Integration Network (LHIN).

The Palliative Care Common Referral Form is completed by your health care provider. It can be faxed directly to the Hospice or sent through your local LHIN. The hospice fax number is 416-963-9466.

Once the form is received, the team will review your application to ensure we can provide you with safe, appropriate care. Once the application is accepted, the Intake Nurse will contact you when a room becomes available.


Applying in advance

If you receive a palliative care diagnosis, we encourage you and your family to tour the hospice as soon as you feel comfortable. This will help you decide whether hospice care is the right fit for you.

You are welcome to apply for admission to the hospice well in advance of your need to stay. We will keep your application on file, and you can contact us when your needs change and you would like to be considered for a room.


Declining admission

We understand that moving into a residential hospice is a big step. You may not be ready to move when admission is offered. If you decide to delay your move, we will fully support your choice. If you wish, we can notify you when a bed becomes available in the future.
